Below you will find parent resources you need to keep you informed with all that’s happening at Providence Christian Preschool as well make sure you have access to all the forms you need.

For each student, Pennsylvania State Law requires the Child Health Report (CD51) with immunization records in its files no later than 60 days after the first day of attendance and the Emergency Contact/Parental Consent Form before the start of the school year.
